Requirements  OPEN LOCUM- Physical Medicine and Rehabilitation JOB DETAILS: _____________________________________________________________________________We are looking for 2 providers to work 7-10 days a month. Providers will work Monday-Friday (8:30am-4:30pm) and on weekends they will work 12hr call with 1.5 hours of patient contact included. Call back should be minimal. Please ensure bids reflect the below: Guaranteed Hours: 8 per day OT to apply after 10 hours per day All overtime should be preapproved by Chris Ryan or the medical director prior to the end of the shift. Please either let them know while onsite or text/email them if possible. REQUIREMENTS: __________________________________________________________Active CA license and CA DEA Must be board certified or board eligible within 3-5 years of residency BLS and ACLS certifications Please note the following travel deviations from our contract will apply to the assignment (or current confirmation agreement):NOT reimbursed:oMileage from provider’s home to departing airport, as well as from airport to home upon return.oRide share (Lyft/Uber/etc.) and/or taxi from provider’s home to departing airport, as well as from airport to home upon return.WILL be reimbursed:oAirport parking at provider’s departing airport if provider drives themselves to the airport.oInsurance policies for rental cars must be pre-approved, prior to booking, otherwise it will not be reimbursed.oRide share (Lyft/Uber/etc.) and/or taxi from destination airport to hotel/lodging for assignment, as well as from loPRIVILEGING: ___________________________________________________________oPlease note that the privileging timeline will not begin until the RFA and application are turned back in. Typical turn around for RFA is 48-72 hours. Application should be submitted within 5 days.oTypical privileging timeline is 90 days oOnce the file is completed, we can look at temporary privilege eligibility. oAll HBS specialties require 2 years of current Hospital clinical activity in the specialty.oGaps greater than 3 months must be explained.oInclude current number of case logs upon presentation for surgical specialties.
