• You’ll practice in a team-based care model —with strong clinical and operational support.
• You’ll build meaningful relationships with patients and families.
• You’ll serve a diverse and underserved population , where your care has visible impact.

Provide full-spectrum, patient-centered primary care:

• Care for adults, children, and GYN patients.
• Deliver preventive care and chronic disease management using evidence-based guidelines (USPSTF, Bright Futures, AAP).
• Review labs and imaging with timely, patient-focused follow-up.
• Participate in team huddles, peer collaboration, and quality initiatives.
• Option to support student/resident training.

• MD or DO from an accredited residency program
• Board Certified or Board Eligible (Family Medicine or Internal Medicine)
• Eligible for or active Kentucky medical license (required prior to start)
• Active DEA
• A passion for community health and health equity
